Please contact Employee Health Service (EHS) by calling Employee Health at 773-878-8200, Ext 1606, Monday – Friday from 8 a.m. to 4 p.m. and weekends from 8 a.m. to noon. When closed follow your normal Call-off procedures and call EHS in the morning. Please do not fill out the EH Questionnaire, but instead please call Swedish EH as directed above.

- * In clinical areas always wear a facemask. When interacting with patients, wear a facemask and eye protection. In non-clinical areas wear a facemask when you are unable to maintain 6ft social distance. *
